New Client Forms

A deposit for the first exam fee ($25.00) will be required to book your first appointment as this reduces no-shows and last-minute cancellations that impact our ability to care for current patients. This is a non-refundable deposit that may be transferred to a rescheduled appointment if reschedule is made 48 hour in advance. Future appointments do not currently require a deposit.

A staff member will reach out to you to schedule as soon as they’ve set you up in our system, usually the same business day.
